To schedule an appointment at Quest Diagnostics:
- Go to the Quest Scheduling page
- You can create or login to a Quest account or "Continue as Guest"
- Enter your personal information
- Select "All Other Tests" as your reason for visiting
- Enter your zip code
- Find a location that is convenient for you.
- Note: It must be specified as "Quest Diagnostics" to ensure you receive our reduced rates.
- Look for an open appointment that works for you!
Be sure to bring your photo ID and insurance card, if applicable, with you to your lab visit.
Completing Labs Out of State or in Oklahoma? You must bring a copy of your lab order to your lab appointment. To request a copy, please send us a message.
