How do labs work with a FOLX subscription?

TL;DR

  • Labs are not included in the membership fee.
  • FOLX gender affirming hormone therapy members usually have labs once or twice a year.
  • We offer our members negotiated out of pocket rates with Quest Diagnostics. In this case, members pay FOLX directly. We bill members for these labs after they go to Quest, typically within a few weeks.
  • If you do not go to a Quest facility to perform your labs, you will need to provide payment in person (either out of pocket or using insurance) when you arrive to perform your labs. FOLX will not bill you if this is the case.
  • Quest locations must be specified as "Quest Diagnostics" and not just any Quest location on their finder.
  • Members in NJ, NY & RI must pay for Quest labs out of pocket due to state laws.
  • Members in ND & SD must find local labs since there are no Quest locations in state.
  • Members who prefer a non-Quest lab should inform their clinical team via the Message Center.
  • The Member Navigation team is always available to help find a lab facility, if needed.
  • You are responsible for the cost of your labs. To avoid surprises, we recommend knowing your lab costs before going to the lab. 
    • Paying out-of-pocket at Quest? Send us a message with any lab billing questions!
    • Using a non-Quest lab? Since labs determine their own prices, we highly recommend contacting your preferred lab before your appointment to confirm their rates. This will help prevent unexpected charges when you receive your bill.
    • Using Insurance? It's always best to check with your insurance company directly to confirm rates and coverage.
