What do I need to prepare for a Wellness and Preventive Care or Illness Care visit?

While a virtual visit with a clinician is convenient, it requires some preparation to ensure it goes smoothly. Here are some things you should do to prepare:

  1. Technical Setup: Make sure your internet or cell phone connection is stable and you have a device with a camera and microphone for video conferencing.
  2. Quiet and Private Space: Find a quiet, well-lit, and private space where you won't be interrupted during your consultation. You may bring a support person to the appointment if you like. That’s totally okay! 
  3. Medical History: Make sure you have completed your full medical history intake form before the visit and included all information. This allows your clinician to review the information and prepare before the visit, instead of spending time on the visit inputting information into your chart. If you were not able to complete your intake prior, make sure to have your medical history ready. This includes any chronic conditions you have, medications you're taking (including doses), and any allergies. (Tip: sometimes it's helpful to bring the bottle so that you can show it to your clinician.) If it's your first time seeing this provider, be prepared to discuss your past medical history in detail.
  4. List of Symptoms and Concerns: You’ll discuss any symptoms or health concerns you have. Consider writing them down ahead of time, so you don't forget anything. Keep in mind that you may not be able to address all your health concerns in a single visit, so start with the issues that concern you the most, typically 1-2 health concerns per visit. And you can always schedule another visit. 
  5. List of Questions: Write down any questions or topics you want to discuss with your clinician. It's easy to forget these during the consultation, so having them written down can be very helpful.
  6. Medical Devices: If you have a blood pressure monitor, glucose meter, thermometer, or any other device that could provide useful health information, have it ready.
  7. Pharmacy Information: If you need a new prescription, have the contact information for your pharmacy handy.
  8. Health Insurance Information: If you're using health insurance, make sure you have updated the information in your FOLX account. 
  9. During the visit, be open and honest with your clinician. Even though you're not in the same physical space, it's important to communicate as if you are. After the visit, make sure you understand the next steps, whether that's a follow-up appointment, tests, or a new medication regimen.
