What if my name on my insurance card is different from the name on my ID for in-person labs? Will my legal name be on my lab orders?

  • Updated February 07, 2023 16:42

We are required to order labs using the name on your legal photo ID (ex. driver's license). You may be required to show this ID when you go in to get your labs done.
