Membership
FOLX members pay a $43.99 monthly membership fee. If it’s their first time signing up, members will have access to a 10-day free trial period. Following the trial, we’ll automatically charge the card on file. The date of the charge will become the billing date for each month moving forward.
Members can also opt into a $349 yearly membership at any time, taking advantage of a nearly $179 savings. Note that first-time members do not receive a 10-day free trial with yearly membership.
Prescriptions
Members receiving medications will have them prescribed based on their unique care plans. Those receiving gender affirming hormone therapy (GAHT) have the option to utilize our pharmacy partner and have their prescriptions shipped directly to them. Members will receive an invoice from FOLX to complete payment prior to their order being sent. Alternatively, members can opt to have their prescription sent to the local pharmacy of their choosing and pay the pharmacy directly.
All non-GAHT medication will also be sent to a member’s preferred local pharmacy where the member will pay the pharmacy directly for their prescription. Members have the option to utilize Capsule if they live in an area of service.
Members utilizing our partner pharmacy for their GAHT prescription(s), will receive an email notification once their clinician has submitted their order, prompting the member to review the prescription and complete payment. Once payment has been submitted, the order will be sent to the pharmacy to be filled. Medication invoices expire after 30 days.
Labs*
FOLX offers self-pay members negotiated rates at Quest Diagnostics, so most members utilize their facilities for lab work. Quest Diagnostics bills FOLX directly to ensure members receive our discounted rates, and FOLX bills members after they go to the lab, typically within a few weeks. Members receive a reminder message a few days prior to being charged.
If members opt to utilize an alternate lab, they'll need to notify their clinical team to have their orders sent there. Members will be responsible for paying the lab directly. Since labs determine their own prices, we highly recommend members contact their preferred lab before their appointment to confirm rates. This will help prevent unexpected charges when members receive their bill.
Likewise if members use insurance to pay for labs, we recommend checking in with their insurance company directly to confirm rates and coverage before completing labs.
Visits*
Members pay for visits at the time of booking, either self-pay rates or copay for members with in-network insurance.

*Important Note -- to ensure we are able to provide you with the safest, highest quality clinical care, your clinician will order labs and require follow up video visits based on your individual care needs. They will review those requirements during your visit, and you will receive reminder emails and messages when your labs and visits are due.
