How will I pay for labs?

For members without insurance or without in-network insurance (paying for labs out-of-pocket)

  • Unless you tell your clinician otherwise, FOLX will order your labs to Quest Diagnostics. Quest Diagnostics bills FOLX directly to ensure you receive our discounted rates, and we pass that cost on to you. 
  • Your card on file will be charged after you complete your labs, typically within a few weeks. We’ll send you a reminder message a few days prior to charging your card. 
  • To avoid surprises, we recommend knowing your lab costs before going to the lab. If you have any questions about your lab costs, please send us a message.

For members using in-network insurance

  • Unless you tell your clinician otherwise, FOLX will order your labs to Quest Diagnostics. You'll provide your insurance at the time of your lab visit. FOLX will not bill you for your labs in this case.
  • It's always best to check with your insurance company directly to confirm rates and coverage before completing your labs.

For members using a lab other than Quest Diagnostics

  • If you do not go to a Quest facility to perform your labs, you will need to provide payment in person (either out of pocket or using insurance) when you arrive to perform your labs. FOLX will not bill you for your labs in this case.
  • Since labs determine their own prices, we highly recommend contacting your preferred lab before your appointment to confirm their rates. This will help prevent unexpected charges when you receive your bill. 
  • Likewise if you're using insurance, it's always best to check with your insurance company directly to confirm rates and coverage before completing your labs.

State-specific Quest exceptions

  • In NY, NJ, and RI, patients must pay Quest directly (due to state laws). FOLX will not bill you for your labs in this case.
  • There are no Quest locations in SD or ND! If you are in one of these states, or anywhere else in the US and would like assistance with finding a non-Quest lab, please send a message so that our Member Navigation team can aid you in that search!
