We’d love to have you return to FOLX for care! You can reactivate your membership directly in your FOLX account by following the steps below:
- Signing in to your account
- Navigating to the “Billing” tab
- Clicking “Reactivate Membership” under the FOLX membership section.
After reactivating, please review your account to make sure your information is up-to-date, such as your address and pharmacy preference.
Important note: If it's been more than 6 months since your last interaction with your clinical team, you'll need to schedule a follow-up visit to restart any of your prescriptions. If it's been more than 12 months since your last interaction, you'll need to schedule an initial visit to re-establish care.
