How do I reactivate my membership?

We’d love to have you return to FOLX for care! You can reactivate your membership directly in your FOLX account by following the steps below:

  1. Signing in to your account
  2. Navigating to the “Billing” tab
  3. Clicking “Reactivate Membership” under the FOLX membership section.

After reactivating, please review your account to make sure your information is up-to-date, such as your address and pharmacy preference

Important note: If it's been more than 6 months since your last interaction with your clinical team, you'll need to schedule a follow-up visit to restart any of your prescriptions. If it's been more than 12 months since your last interaction, you'll need to schedule an initial visit to re-establish care.
