How do I send medical documentation to my care team?

The following steps can be followed when you need to send documentation regarding your care -- such as medical records, lab results, and referral forms -- to your care team.

  1. Take a picture of all of your documentation if it’s in physical form.
  2. Log in to the Message Center.
  3. Click the Send a Message button.
  4. Select My Prescription & Care from the What can we help you with? drop down. 
  5. Select I have an update to my medical history option from the Select the option that best fits your request drop down.
  6. Under Attachments click Add file or drop files here.
  7. Click Submit.
