The following steps can be followed when you need to send documentation regarding your care -- such as medical records, lab results, and referral forms -- to your care team.
- Take a picture of all of your documentation if it’s in physical form.
- Log in to the Message Center.
- Click the Send a Message button.
- Select My Prescription & Care from the What can we help you with? drop down.
- Select I have an update to my medical history option from the Select the option that best fits your request drop down.
- Under Attachments click Add file or drop files here.
- Click Submit.
