Does FOLX provide letters for gender-affirming surgeries?

Yes, FOLX offers letters for gender-affirming surgeries. To request a medical letter for insurance or surgeon approval, send a message in the Message Center (Click Send a message in the upper right hand corner).

At this time, a letter from a mental health professional is only available to members in select states. In those states, providing a letter for gender affirming procedures is up to the discretion of our therapists. If you live in one of those states, you can schedule a visit with one of our therapists. If you do not live in one of the states listed here, you can send a message in the Message Center for a referral to an affirming provider.

Please note: a therapist's role in letter writing is to evaluate and attest to documenting a member's eligibility and readiness for medical treatment. Because each person is unique, one of our therapists may need to meet with you more than one time to gather enough information to make this assessment. Your therapist will communicate this with you directly.
