Minnesota Testosterone Requirements

Overview

Due to Minnesota Regulations Minn. Stat § 151.37 and Minn. Stat. § 147.033, in order for you to receive Testosterone prescribed through telehealth, we need you to:

  • Have an in-person clinician refer you back to FOLX for continuation of GAHT treatment. 

FOLX does not have in-person locations in Minnesota, but we will help you find a clinician near you.

 

Getting Started 

  1. Sign Up for FOLX and Book a Virtual Visit.  Once you book your FOLX virtual appointment, our Member Navigators will contact you with a list of affirming clinicians in your area to choose from. You may also choose another in-person provider if you already have one in mind.

  2. Book an in-person visit with a clinician near you. 

  3. Attend your FOLX virtual visit. Your FOLX clinician will walk you through the referral process, work with you to determine your care plan and document the medications to be ordered as soon as we receive your uploaded referral form. 

  4. Download your referral form (below) to bring to your in-person visit for your in-person clinician to sign.

  5. Attend your in-person visit where you’ll get your referral form signed.

  6. Upload your signed referral form using these instructions. As soon as we receive your signed referral form, your Care Team will proceed with ordering your medications. 

Please note:  

  • If you already have an established local provider, they may not require that you attend a visit to complete your referral form. Please contact your established local provider for instructions on how you can submit the referral form for them to complete and fax to FOLX at 833-449-4242.

  • You may attend your initial virtual visit with FOLX without your referral form, but you will not receive a testosterone prescription until we receive a copy of the signed referral form from your in-person visit.
