How do I reactivate my membership?

We’d love to have you return to FOLX for care! You can reactivate your membership by going to folxhealth.com and:

  1. Signing in to your account
  2. Navigating to the “Billing” tab
  3. Clicking “Reactivate Membership” under the FOLX membership section.

Once your account is reactivated, please reach out to your clinician via the Athena patient portal. Your care team will let you know if they have any questions for you in order to get you restarted.
