FOLX offers our members a negotiated rate to Quest Diagnostics and so sends most of our members to one of their facilities for labs. Members have the option to pay FOLX directly (out of pocket) for their lab fees, which you will then be charged for on your monthly billing date that follows the date your lab order was sent to Quest. 

If you have insurance, and would prefer to use it to pay, you can do so when you arrive at the testing facility to get your labs performed.

If you do not go to a Quest facility to perform your labs, you will need to provide payment in person (either out of pocket or insurance) when you arrive to perform your labs. FOLX will not bill you on top of your membership if this is the case. 

There are some states that require the patient to pay directly to Quest instead of charging FOLX (NY, NJ, RI) and some states that do not have Quest locations (SD, ND)  Review this article on lab exceptions for more information.
