For all services, we need to know your age, medical history, surgical history, family medical history, any existing medications you're taking and any allergies you have. We also need an ID and selfie from you for safety and security purposes. FOLX does not serve members under the age of 18 currently.

For gender affirming hormone therapy we’ll also ask if you’ve taken any GAHT medications before, your current anatomy, and any goals you’re looking to achieve with gender affirming hormone therapy. This allows us to build a customized care plan with you and gives you an opportunity to discuss any questions or concerns you might have.
